Current healthcare systems are ill-equipped to support patient self-management of chronic conditions, resulting in fragmented care, reliance on healthcare providers, and low patient adherence to treatment plans. TREAT aims to empower patients through a unified, patient-centric platform that enhances self-efficacy and health outcomes while reducing the burden on healthcare providers.

Key Objectives:

– Create a Unified Data Platform: Develop a semantic interoperability system to integrate patient data from wearables, medical devices, and journals into a standardized format.

– Empower Patient Self-Management: Use AI-driven algorithms to provide personalized recommendations, improving health outcomes with up to 95% accuracy during the project.
